HERPES LABIALIS: COLD SORE, FEVER BLISTER ,TESTs AND TREATMENT

Hi , Welcome readers Herpes labialis or cold sore is another common skin viral infection. It is caused by HSV-1 (Herpes simplex virus-1). It is also called as fever blister. In this article i will try to explain about its sign ,symptom, tests, diagnosis and treatment. 



WHAT IS HERPES LABIALIS 

Herpes labialis or oral herpes is caused by HSV-1 and some times due to HSV-2 too. It is viral infection. Blister like painful lesions appear over and around lips. It is a very recurrent condition and appears again and again even after proper treatment. 

SYMPTOM, SIGNS, TESTs

There is fluid filled blisters which come after redness and pain over or round lips. Initially blisters are firm but after few days it get rupture and scab, crust formed. In absence of treatment pain and lesion remains for long time although it is self limiting and resolved itself after many days. Clinical diagnosis by dermatologist is enough to identify the disease due its peculiar presentation. Still HSV IgM ,IgG  antibody detection test are there to confirm the diagnosis.

        HSV-  IgM - It detects the acute infection and helpful to diagnose present HSV infection.
        HSV- IgG  - It does not detect present infection. It is positive (or negative)
                         It has nothing to do with acute herpes infection. Only if IgM positive shows acute infection. 

IgG shows the immunity and chronic infection of body means patients had such infection in past (may be 10 year or 10 month back) . It is like memory of old infection.

WHAT TRIGGERS HERPES?



Virus remains inactive in nerves and because of stress or any other triggering factor re-activate the virus . Due to reactivation virus comes out in skin and manifests as cold sore or oral herpes. Physical or mental stress,  Sunlight, sunburn, fever, dehydration, or local skin trauma. Surgical procedures such as dental or neural surgery, lip tattooing, are also common triggering factors. 

TREATMENT

Oral acyclovir, valacyclovir or famciclovir are main treatment choice. Acyclovir creams with antibiotic creams should be applied. To prevent the recurrence acyclovir with low dose can be continued for long time.

Hello Friends Hair transplant is one of the much talked procedure at present. Now people are getting aware of this procedure and ask lot of questions to me. As we all know androgenic alopecia is a hormonal problem and difficult to treat medically. Once the person become bald hair transplant is the only method to get hair back. Today in Video I would try to give some general idea about FUE procedure .

After giving local anesthesia in donor area we do the extraction with the help of motor punch. It is painless, suture less, scar less technique and takes proper skills. We loose the follicles with the help of punch which is shown in this video. Puch size depends on graft thickness, tightness and easiness of extraction. Usually we take .8 or .9 mm size.


Once we loose the graft we do plucking of the graft. Means we take out all the graft . We only implant the graft which are having roots. So extraction needs proper skills and expertise other wise graft damage is more.


Once all required grafts are taken out . We start implantation.  Density is the most important thing which we keep in mind while implantation. Graft should not be either very close or very far. Hairline should be more dense than rest of the implanted area.

How to Treat BROWN SPOTS on FACE:Melasma (झाईयां)

Hello Friends, Brown spots over face , around nose, forehead, over nose is very common in dark skin. Today I will try to cover all important points and F.A.Qs related to Melasma or brown spots.

WHAT IS BROWN SPOTS (Melasma) ?

Brown spots or melasma are brownish spots over face , around nose, forehead, over lips, cheeks, below eyes. These spots are very stubborn and very difficult to treat. This is symmetrical present both side of face. It can be either deep or superficial depending on duration and age of patients. Pregnant women are very prone to get such marks due to hormonal disturbance. This is called Chloasma or Mask of pregnancy. 

WHAT ARE CAUSE OF MELASMA ?

There are mainly two factors which cause melasma 1. Sunlight and 2. Hormonal imbalance(pregnancy cause hormonal imbalance). These two are main culprit but other factors further add the disease progression. Other than this thyroid problem, drugs, cosmetics, oral contraceptive pills etc also can cause melasma. 

WHO GETS MELASMA 

1. People who work lot in sunlight , outdoor work
2. Pregnancy 
3. More common in women than men
4. Age 20-40 years
5. Indian skin
6 Family history

TREATMENT

Hydroquinone, Azelaic, kojic acid

These are antipigmentation chemical. These come in market with various combination and strength. All such chemical should be applied in the night because these reacts with light so safer to use in night.

Retinoic acid, steroid and triple combination ( meladerm, melas, etc)

There are so many products in market which are sold with out doctor prescription. These creams could be very dangerous to skin. Steroid cause lot of side effects. (read about steroid ) . Retinoic acid cause peeling, redness, buring, itching sensation of skin. So these creams should be used very consciously. 

Vit.C Serum and multivitamins 

Vit.C serum and multivitamins are add on factor in treatment. It gives better look, nutrition and improvement. (read more about Vit C serum )

CHEMICAL PEEL

Glycolic peel is one of the main treatment option. This gives better results as well as maintenance peel prevent recurrence of melasma.(read more about chemical peel

Q-Switch, Fractional laser

Its use is controversial . Many studies are not in favor of this treatment due post laser pigmentation. I personal have used in few patients and results are very much variable. Few got satisfactory results but few did not get any results. (read about laser hair removal)

Melasma is very challenging condition and very difficult to remove completely. Recurrence is another problem. Most of the time it comes back even after total disappearance . So we still searching a treatment option which can cure melasma completely.
