JOCK ITCH: Itching over groin(दाद ,खाज, खुजली)

Another very common disease specially in warm places where sweating is more. It is caused by fungus. Wetness . sweating, unhygienic condition further increase the infection. In this article I would try to explain its sign and symptoms, precautions, treatment and other important things in simplified manner.  

WHAT IS JOCK ITCH OR RINGWORM (दाद,खाज ) 

It is a fungal infection which affects groin , inner side of thigh , genital area, and may spreads to other part of body too. Skin fold like groin is more prone to get fungal infection due to excessive sweating and moisture. It is also called ring worm. There is appearance of ring like rashes, itching, scaling in groin and other part of body. 

SIGN and SYMPTOMS

I explain it point wise manner

1. There is itching specially when area becomes moist and wet. Itching leads to scratching of area which cause erosions, wounds.

2. Ring like reddish rashes, eczema like lesions, coins like rashes appearance in groin area, thigh , genital or other part of body.

3. There could be folliculitis, pus filled infection due to secondary infection.

4. Some times lesions become scaly and dry. 

5.It is very recurrent condition many times even after treatment it comes back again and again so hygienic condition and precautions are must to prevent it. 

WHO GETS RINGWORM (JOCK ITCH)

1. Diabetic , obesity ,  thyroid patients are more prone to get fungal infection . Recurrence is also more in these patients. Treatment resistance also develops more in diabetic patients. 

2. HIV and other immunity related disease makes patients more susceptible to fungal infection.

3. Sweating , wetness, moisture, uncleaned undergarment, unhygienic routine make condition worse and un-treatable.
4. Poor medications, steroid therapy, drug abusers are predisposing factors for fungal infection

5. Irrational use of Steroid creams like batnovate, ring guard etc increase the resistance 

6. People living in rainy, moist, warm area are more prone to get fungal due to excessive sweating and wet undergarments. 

IS THIS SERIOUS CONDITION ?

No, not at all. If treatment taken seriously and completely It is easy t treat the fungal infection. But In wrong treatment because of self treatment by patients worsen the condition and make the fungal resistant which becomes very difficult to  treat. 

PRECAUTIONS 


1. Dont keep the area wet at all . Use powder frequently to keep it dry

2. Avoid itching and don't scratch it
3. Change the under garments
4. Don't apply general or steroid cream like betnovate etc
5. Don't stop the medicine once you cure. Continue medicine for long time to completely kill the fungal
6. Avoid excessive sweating and wet clothes
7. Check diabetes and thyroid it is really recurrent even after proper treatment.
8. Reduce weight and avoid junk food. 

TREATMENT 

There are multiple creams and oral tablets present . But we should start with basic medicine like fluconazole and ketoconazole. But if these basic medicine do not work then we have stronger medicines too. Here i will mention few medicines which are used frequently in fungal infection.
1. FLUCONAZOLE 150 MG TWICE IN A WEEK
2. TERBINAFINE 250 MG EVERY DAY
3 . ITRACONAZOLE 200 MG ONCE OR TWICE IN A DAY
4. GRISOFULIVINE 375 MG EVERY DAY 
5. TERBINAFINE CREAMS, KETOCONAOLE CREAM, LULIFINE CREAM  etc. 
As per clinician experience we can try all above medicine and get good results.

SKIN Manifestations in DENGUE FEVER

Hello Friends Dengue fever is very common in rainy season and unhygienic environment. It is very fatal disease and delayed identification of disease further increases its fatality. Fever, flue like symptom, joint pain, cough etc are initial symptoms of dengue fever. Patient does not take these symptoms very seriously and unknowingly worsen the condition. Skin manifestation in dengue may help in early detection and treatment. In this article I would try to explain here only about skin related features which can be present in Dengue fever. 

Fever Exanthema 

It comes with in 24-48 hr of fever. According to studies 50-80% patients get Exanthema. It is mild generalized redness of face, back and chest. It is thought to be the result of capillary dilatation. It is easily visible by blanching(putting pressure ) the skin. 

FEVER RASHES 

Appearance of generalized (except palm and sole) rashes all over body. It comes after 2-3 days of fever. Involvement of area increases with time and rashes start joining each other and uninvolved skin looks like white island in red sea. As fever goes off rashes also resolve itself. 





PURPURA, PETECHIAE, ECCHYMOSIS 

These are skin manifestation of dengue due to bleeding. As we know that dengue cause inability of blood to form clot so in severe untreated stage bleeding starts. In skin too blood gets collected and form bruise like skin lesion which is called purpura or petechiae. If size of bruise is big it is called ecchymosis. 

                     These are skin conditions which may come in dengue fever but it is not necessary that these symptoms always come. Many times dengue can be present with out any skin manifestation too. Proper check up is must if dengue is suspected.
